Therapy Directory

We’re building a directory of local therapists and mental health practitioners so that, when people receive their stipends, they have vetted sources to turn to.

Care Measured brief PHQ4 screening for anxiety and depression

Are you interested in a free self-assessment that screens for anxiety and depression? Look no further!

One of our partner therapists, Dr. Orlando Wright, LCSW-C, Founder of Halcyon Clinical Services, developed an app called Care Measured.

This 7-question survey will help you determine where you are in terms of needing mental health support. In the first portion, the PHQ4, here is the scale:

  • Normal is 0-2

  • Mild is 3-5

  • Moderate is 6-8

  • Severe is 9-12

This is a required resource for anyone looking to receive stipends from us and we thank Dr. Wright for this tool!
